Offloading & Preventive Surgery
Surgery that relieves pressure points to prevent ulcers returning.
Diabetic Foot Offloading Surgery in India
Diabetic foot offloading surgery in India is a group of small procedures that permanently relieve the pressure points causing an ulcer so a wound can heal and is far less likely to come back. At EDFC, Dr. Ashutosh Shah uses these when insoles and footwear alone can't take enough pressure off a high-risk area.
What is offloading surgery?
In a foot with neuropathy, repeated pressure over a bony point builds a callus and then an ulcer. Offloading surgery changes the foot's mechanics so that pressure point is relieved for good. Common procedures include Achilles tendon lengthening (to reduce forefoot pressure), toe tenotomy (to straighten a clawed toe that rubs), and bone procedures such as removing a prominent bit of bone (exostectomy) or a metatarsal head.
It is called preventive because it tackles the cause, not just the wound. When a specific deformity keeps producing ulcers at the same spot, correcting the mechanics is often what finally allows healing. At EDFC India's first chain dedicated to foot care these procedures are performed by Dr. Ashutosh Shah (M.S., M.Ch.) in Surat (Gujarat) and Vizianagaram (Andhra Pradesh), usually after a pressure & biomechanical assessment.
Who needs offloading surgery?
- An ulcer over a pressure point that won't heal with insoles alone
- A wound that keeps returning to the same spot
- A clawed or hammer toe that repeatedly rubs and ulcerates
- A tight Achilles tendon increasing forefoot pressure
- A bony prominence creating a high-risk pressure zone
When to see a doctor in India: if a callus or ulcer keeps coming back despite good footwear, ask whether a small offloading procedure could solve the underlying cause.
Common offloading procedures
- Achilles tendon lengthening — reduces high pressure under the forefoot.
- Toe tenotomy / flexor release — straightens a clawed toe that ulcerates.
- Exostectomy — removes a bony prominence under an ulcer.
- Metatarsal head resection — relieves pressure under a stubborn forefoot ulcer.
- Post-op offloading — protected footwear or cast while the foot heals.
