Midfoot Collapse

Midfoot collapse is a flattening and deformity of the arch, often the result of Charcot changes in a diabetic foot. It creates a bony prominence on the sole that ulcerates easily.

Midfoot collapse is the falling of the foot's arch so that the middle of the sole bulges downward — often a result of Charcot foot — creating a "rocker-bottom" shape that is highly prone to ulcers. At EDFC, midfoot collapse is stabilised with offloading and bracing, and reconstructed surgically when the foot is unstable or ulcerating.

What is midfoot collapse?

The midfoot normally forms a supportive arch. When the bones and joints there weaken and give way — most often through Charcot foot in diabetes — the arch collapses and the middle of the sole drops, sometimes into a rounded "rocker-bottom" shape. This puts abnormal pressure on a part of the foot not designed to bear it, which readily breaks down into a serious ulcer.

Signs to watch for

  • A flattening or dropping of the arch
  • A bony bulge under the middle of the sole
  • A "rocker-bottom" foot shape
  • A callus or ulcer under the midfoot
  • Swelling, warmth, or instability in the foot

When to see a doctor in India: a changing foot shape or a sole that bulges needs prompt assessment — especially with diabetes, where this often signals Charcot foot and a high ulcer risk.

Why it happens

The most common cause in diabetes is Charcot neuroarthropathy, where nerve damage lets the midfoot bones fracture and shift under normal walking loads. Less commonly, it follows injury or advanced arthritis.
