What is Buerger's disease?

Buerger's disease causes inflammation and clotting in the small and medium arteries (and sometimes veins) of the limbs, narrowing and blocking them so blood can't reach the fingers and toes. It almost always occurs in people who use tobacco, typically younger adults, and is more common in parts of Asia including India. Unlike peripheral artery disease, it affects smaller, more distal vessels — and its progression is tightly tied to continued tobacco use.

Signs to watch for

  • Pain in the hands or feet, including at rest
  • Pale, blue, or cold fingers and toes
  • Painful sores or ulcers on the fingers or toes
  • Colour changes in fingers/toes with cold (Raynaud-like)
  • Darkening of a fingertip or toe (tissue loss)

When to see a doctor in India: a tobacco user with painful, cold, or discoloured fingers or toes, or non-healing sores on the digits, should be assessed promptly — early diagnosis and quitting tobacco can save fingers and toes.

Why it happens

The cause is closely linked to tobacco in all its forms — smoking, bidis, and chewing tobacco. The vessels react with inflammation and clotting that blocks blood flow. Without stopping tobacco, the disease typically progresses despite other treatments.
